What is M+D ProHealth?

M+D ProHealth has assembled an advisory panel to guide and evaluate the ongoing project.
M+D ProHealth is dedicated to improving the quality and availability of orthopaedic and spine care.
What to expect before your appointment
- Once we have the referral from your family doctor we will contact you with an appointment date and time.
- We ask that you secure a copy of any recent diagnostic images on CD disc (MRI, CT Scans) from the facility that performed them (unless it is St Joseph’s Hospital, Hamilton as we can access them directly). These can be obtained by calling the hospital’s diagnostic imaging department and informing them of your appointment with a specialist. They can often get have the disc ready for you within a few days for pick up. You would bring this disc with you on the day of your assessment.
- Your appointment will be made either in Hamilton or Oakville. Please make sure that you can arrange transportation to and from the appointment.
- Please arrive 15 minutes before your appointment to complete necessary paperwork.
- Please be prepared to pay your assessment fee at the time of the visit. We will then issue you a receipt for your records or for submission to a claims adjuster or insurance company.
- Please be advised that M+D ProHealth does not submit or facilitate claims to WSIB, Third Party Insurance or any other adjuster on behalf of the patient.
What to expect after your appointment
- If diagnostic imaging is required for further consideration, we will facilitate the requisition for local residents. If you are from out of town, we will ask your referring physician to arrange the necessary tests. You will be notified when an imaging date has been set by the hospital. We will either call you directly with this date, or the hospital will get in touch with you. Please be advised that it may take a month or more to have a confirmed imaging date.
- A dictated note will be sent to your family doctor 2-3 weeks after your visit.
- If we need to see you in follow up, we will make this appointment within 2 weeks of your assessment or imaging.
